Your child's medical record
The Child and Adolescent Health Service (CAHS) is made up of four service areas:
- Neonatology
- Community Health
- Child and Adolescent Mental Health Services (CAMHS)
- Perth Children’s Hospital (PCH)
Your child may receive or be receiving care from more than one CAHS service area.
Each clinician your child sees will make notes about the care provided to your child. This will be secured in the paper, digital or electronic medical records used in that service area.
Your child’s personal information is regarded as strictly confidential.
It will only be shared outside of CAHS with your permission or if the law requires it to be disclosed.
Where it is relevant, CAHS staff can share patient/client health information within and across CAHS service areas.
Working together will help us provide safe, high quality and timely health care to your child.
It is important that your child and family feel comfortable with the care that you receive from CAHS.
If you have any questions or concerns about how your child’s health information is handled, talk to a clinician involved in your child’s care.
If you don’t feel that your question or concern has been addressed, you can provide feedback through our Consumer Engagement Team.
You can ask to see the information we hold about your child by contacting our Release of Information department.
