Community Health
We offer all families five free child health or Purple Book appointments with their community child health nurse at:
- 0-14 days
- 8 weeks
- 4 months
- 12 months
- 2 years.
The first contact is made by a local Community Health Nurse before a new baby is 14 days old. Subsequent appointments are made by parents here.

All clients must meet all of the following eligibility criteria:
- the child must be under the age of 16 years
- have an eligible Medicare card/number (clients with yellow Medicare cards are ineligible)
- live at an address in the Greater Perth Region.
Child development services for children living in regional areas are available from local community health centres through the WA Country Health Service.
Clients who, at any point in time, no longer meet eligibility criteria will be discharged.
The clinician(s) will discuss this with the parent, and agree on completion of current service plans, and referral or handover to alternative services.
Making a referral
A referral can be made by parents, legal guardians or professionals. All referrals require parent or guardian consent.

Eligibility criteria
The program is offered to all children and adolescents who meet the following eligibility criteria:
- aged 5½ to 18 years
- adequate family/carer support to engage with the program
- daytime urine and bowel continence
- no ongoing issues relating to constipation or other comorbidities
- a nocturnal enuresis bedwetting pattern of more than twice weekly.
Referral form
Email all referrals using this Nocturnal Enuresis Referral form to the Community Health Enuresis Program.
The Neonatal Special Referral to Child Health Services Form is used as a referral from maternity services to community health services across the state where STORK is not available.
It is used to signal and prioritise families and infants with risk factors identified in the antenatal or postnatal period.
The Form can be completed:
- electronically as writable PDF (saved and sent in an email) or
- downloaded, printed and filled in (and then scanned and emailed).
The completed Form is to be sent via email (as an attachment) to the appropriate address (details available on form).
