Our Model of Care
In 2023, we worked closely with Aboriginal families and community, our staff and local organisations to look at how we could better provide a culturally responsive and secure service that supports Aboriginal families to raise happy, healthy children.
From this, we developed the Aboriginal Health Team Model of Care. This shows how we provide our services with Aboriginal Culture at the core. Find out more about the Model of Care artwork.

What to expect
You can choose the type of care you want and who provides it.
- We are notified when an Aboriginal child is born in the Perth metropolitan area.
- After birth, the local Aboriginal Health Team offers metro Aboriginal children and their family a home visit.
- We offer health services from birth to 5 years.
- You can choose care services from:
- the Aboriginal Health Team
- your local child health centre
- another service.
Child Health (Purple Book) appointments
- We offer 5 child health checks at 0-14 days, 8 weeks, 4 months, 1 year and 2 years. (Learn more about what happens at each appointment.)
- Based on your family’s needs, we may offer extra child health checks at 4 weeks, 6 months, 8 months, 18 months, 2½ years, 3 years, 3½ years and 4 years.
- You can contact our centres during opening hours if you’re worried or have any questions.
Want an appointment?
- We will contact you by phone within the first 14 days of your child’s birth to offer the home visit.
- You can contact us for self-referral.
- Or you can ask your child health nurse and they will contact us for you.
