Online questionnaires

The Child and Adolescent Health Service, Child Development Service uses online questionnaires from three outside suppliers. 

These questionnaires help us with your child’s assessment and treatment.

The suppliers collect and store all your answers to the questions in the questionnaire, including personal information, in secure systems in Canada or the USA. 

The suppliers provide us (the Child and Adolescent Health Service) with access to your questionnaire responses and we store a copy on your child’s medical record. This information will be treated with the same security and privacy requirements as any other information on your child's medical record.

Even though they are overseas companies, the suppliers must follow Australian privacy laws, as well as their own national laws. Apart from providing the information from the questionnaires to the Child and Adolescent Health Service, the suppliers will not provide any information or data from the questionnaire to anyone else unless they legally have to.
